Physical Therapist – Outpatient Rehab

Position Overview:

The Physical Therapist is responsible for evaluating, planning, and providing treatment for patients referred to the service. The role involves both individual and group therapy for patients with various diagnoses and symptoms.

Essential Duties & Responsibilities:

  1. Assess and interpret diagnostic data while considering patients’ current conditions and medical history.
  2. Develop and implement patient-specific treatment plans and goals.
  3. Supervise and direct patient treatment activities of Assistants as per department protocols.
  4. Ensure timely initiation of patient care activities and adherence to treatment schedules.
  5. Complete accurate and timely documentation in compliance with departmental and regulatory guidelines.
  6. Accurately capture and submit patient charges within 24 hours of treatment.
  7. Maintain competency in program specialist areas (In-Patient and Work Hardening, if applicable) and complete annual competencies.
  8. Demonstrate flexibility and competency in cross-training as required by department needs.
  9. Assist with onboarding and orientation of new staff and students.
  10. Perform additional duties as assigned.

Qualifications:

  • Education: Bachelor’s degree in Physical Therapy required.
  • Licensure: Current Ohio PT license.
  • Experience: Minimum of one clinical rotation in a Rehabilitation Services setting (Acute Rehab, SNF, Acute Medical, Outpatient, Home Health, or Behavioral Health).
  • Skills:
    • Strong attention to detail.
    • Effective problem-solving and assessment abilities.
    • Excellent written and verbal communication skills.
  • Additional Requirements:
    • Valid driver’s license and proof of auto liability insurance (for HomeCare).
